Claim procedures
The underwriter must be notified of all claims within 6 months after the member’s date of death. Even if all documents are not available by the funeral home.
The underwriter reserves the right to request further documentation from claimant/representative in order to properly assess a claim.
➢ On claims stage the following documents must be submitted:
Certified Electronic Death Certificate.
Certified Copy of principle members ID.
Certified copy of deceased member’s ID
BI1663 (Notice of death)
Completed mandate to pay the undertaker signed by the policy holder or main member.
Completed claim form.
Signed and dated application / membership form.
In case of unnatural cause of death, a completed police report.
48 HRS payout -Claim must be received before 11h00am
6 Months slips/if no proof of underwriting was provided on inception.
Notes
➢ Should any fraudulent claims be submitted will the underwriter be under no obligation to pay the claim.
➢ The underwriter may choose to cancel the contract if any fraudulent claims are submitted.
➢ No claims will be paid on any arrears premiums due to the underwriter.
➢ All claim documents must be clear and visible
➢ Claims can be faxed /emailed or delivered to our offices.
